Treatment for Mental Health, Alcohol, Drugs & Other Addictions

If you are a Jefferson County resident looking for outpatient mental health or addiction treatment, you may access services by calling 920-674-3105 and asking to speak to someone about starting treatment.  Our staff will assist you in navigating “open access” or same day access to treatment.

In lieu of calling our office, you also have the option of completing the paperwork found below to get started with the process.  Once completing the paperwork, one of our staff will call you to discuss next steps.  At any time during the process of requesting services or filling out paperwork, please call us at 920-674-3105 with any questions.
