Open Records

Open records information is available Monday through Friday from 8:00 a.m. to 5:00 p.m.

To request records:

  • Review the "Records Request Form for Requestors"
  • Complete and submit the "Limited Record Information Request Form".    Please provide as much information as possible such as incident number, Citation number, individual(s) involved, date, time, location, deputy's information, type of incident, etc.  Please indicate what records you are requesting in detail (incident report, attachments, citation(s), photos, recording, etc.). Include your contact information as well and date and sign the form. 
  • If you want to authorize another individual/organization to pick up records on your behalf, please complete the "DPPA Release of Personal Information Form".  As you will note, this form requires your signature to be notarized.
  • To obtain medical records please fill out and submit the Medical Records Release Form.
You may submit your request in person, by mail or via FAX at 920.674.7126.  If you have additional questions, please contact the Records Department at 920.674.7354.

Please bring exact change, if possible, when paying for and picking up your requested records.
 
Failure to respond within 30 days after being notified that your record request is complete will result in said request being marked "No Response" and your request will be filed in our archives.  If at any time following said 30 days the record is desired, a new record request must be submitted to the Records Department.  Each record request will be reviewed and responded to accordingly. 
 
It is our goal to respond to open record requests within 10 working days.
