JCHD will be administering COVID-19 vaccine at the health department Mondays through Fridays from 8:30AM to 4:00PM by appointment only.
Appointments must be made by calling the health department at 920-674-7275. Capacity may be limited and only certain COVID-19 vaccine brands may be available due to ordering and storage constraints, and to reduce waste.
