ALZHEIMERS FAMILY CAREGIVER SUPPORT PROGRAM (AFCSP)
The Alzheimer's Family Caregiver Support Program is available to help families caring for a loved one who has been diagnosed with Dementia. The AFCSP provides services and funding for Alzheimer's patients and their caregivers. The purpose of the program is to provide opportunities to maintain persons with dementia in community placement. Eligible individuals can receive a small grant annually to help pay for needed services and supplies. Eligible individuals or households may receive up to $4,000 per year to help cover the cost of day care, respite care, home health services, etc.

Who Qualifies?
Individuals or households with an income of less than $40,000 per year who have a diagnosis of Alzheimer's disease; Huntington's Chorea; Irreversible Multi-infarct Disease; Parkinson's Disease; Pick's Disease or any irreversible dementia.

SafeReturn is a program offered through the National Alzheimer's Association. SafeReturn helps identify, locate and return "wandering" individuals who are memory impaired due to Alzheimer's disease, through a jewelry identification program.
There are several benefits to the Safe Return Program. First and foremost, all program participants are registered in a national database that includes emergency contact information and a photograph to help reunite lost individuals with their caregivers no matter where they live. A nationwide 24-hour toll-free crisis line is available 24 hours per day, 365 days per year to handle calls when an individual is lost or found. Individuals are initially identified through various identification products, which they wear or keep in their wallets. All products are inscribed with the toll-free crisis number and contain information that alerts others of the individual's memory impairment.
Registering for the program is easy and can also be done by contacting the ADRC. There is a nominal one-time registration fee of $40 that includes identification products. For those who are unable to pay the fee, some funding may be available to help cover the cost. Please help keep your loved one safe by registering now.

Mission Statement: To serve and support caregivers by connecting them to community resources to enable them to provide compassionate, dignified care to their loved ones within their own homes.
The goal of the Jefferson County's Family Caregiver Support Program is to support caregivers in their roles and keep Jefferson County residents as independent as possible in their community.
Our program maintains and improves the ability of the caregiver in the home by coordinating formal and informal support services. The intent of this program is to provide information, support and assistance to family caregivers who are caring for a person 60 or over.
Services include but are not limited to: personal care, information and education, adult day care, nutrition counseling, transportation, temporary respite, emotional support and advocacy.

Caregiver Support Group The Aging and Disability Resource Center facilitates a support group on the second Wednesday of each month at 1:00 p.m. Group meetings are held at the ADRC, 1541 Annex Road in Jefferson. If you are caring for an aging relative, or are a senior citizen who is caring for a spouse, disabled adult child or grandchild, please consider joining us. Please call ahead to 920-674-8136 to reserve a space as group size is limited.

Coping with Challenging Moments is designed to teach you skills to deal with challenging behaviors while caring for people with disabilities. You should attend if you care for people with:
