Each consumer is assigned a case manager who will coordinate services, and collaborate with the consumer to form a treatment plan. Case management services include offering a variety of organizational, community, and clinical supports.
Treatment Planning & Delivery
Each consumer formulates with their case manager a treatment plan each six months. Treatment is delivered within the CSP team. It is individualized and goal oriented. It may include such services as symptom management, supportive counseling, individual and/or group psychotherapy, and medication. A team of professionals from the CSP are assigned to provide treatment.
The CSP offers a variety of active treatment groups, community resource groups and recreational activities.
Dr. Mel Haggart is the psychiatrist for the CSP and the Medical Director. He meets with consumers as often as necessary (i.e. from 2 times a week to every other month) for their success. He follows consumers very closely including throughout hospital stays. This minimizes fragmentation of psychiatric care and facilitates close medication monitoring.
The CSP works collaboratively with a benefit specialist from Jefferson County Income maintenance unit. We also have four CSP workers who are trained benefit specialists. They are highly educated and proficient in:

CSP consumers report this assists them with keeping their benefits intact, in understanding them better, and making more informed choices.
CSP staff are able to assist consumers with transportation to medical appointments and employment. CSP staff also are able to help consumers access the volunteer driver system of Human Services.
CSP staff work collaboratively with Jefferson County Human Services Intake Unit to provide 24 hour crisis intervention. A CSP staff can be reached at all times.
