Transportation Program 

                                    
The Human Services Transportation Program is intended to provide Driver/Escort Services to the elderly and persons with disabilities to get to medical appointments. This program is only available to those individuals who have no other means of transportation.

  • A $ 1.50 co-pay is requested per one way trip in county transportation.
  •  A $ 7.50 co-pay is requested per one way trip out of county transportation.
  •  Those unable to pay the fares may apply for a co-payment waiver.

Mobility Manager

Mobility Managers help older adults, people with disabilities and anyone facing barriers finding transportation. Mobility Managers focus on meeting individual customer needs through a wide range of transportation options and service providers.

A Full-Time Mobility Manager is here to assist you in Jefferson County.

Medical Transportation Management (MTM)

MTM provides rides for Non-Emergency Medicaid and BadgerCare Appointments. Rides to routine appointments may be scheduled from 7:00 a.m. to 6:00 p.m. Monday - Friday. Rides to urgent appointments may be scheduled 24 hours a day, seven days a week. For further information regarding Non-Emergency and Urgent appointments contact the MTM Reservation Line: 1-866-907-1493. If you need assistance in scheduling your rides with MTM, please contact the Mobility Manager at 920-675-4049.

Veteran’s Van
Veterans Van

The Veteran’s Administration has provided Jefferson County with a van to take vets to medical appointments at the VA Medical Center in Madison. Service is available Monday-Friday and rides are scheduled on a first-come, first-served basis.

Rides are free of charge to Veterans.  To request a Driver, please call: 920-674-8104


Volunteer Opportunities

Volunteer drivers are always needed to transport passengers to medical & agency appointments. Drivers receive mileage compensation and are eligible for trip per diems. To inquire about becoming a Volunteer Driver, please call 920-674-8104 and ask for the Transportation Coordinator.

Wednesday Walks

The "Wednesday Walks" program offers a wonderful way to get fresh air, feel the sun, exercise, meet new people and enjoy many unique attractions in Jefferson County. Each trip includes a walk and an optional lunchtime meal at one of Jefferson County's senior dining sites. Reservations are required. Please call: 920-675-4049.

The Driver / Escort and the Mobility Manager programs are funded in part by the Federal Transit Administration (FTA) as authorized under 49 U.S.C. Section 5310 Mobility Options of Seniors and Individuals with Disabilities Program (CFDA 20.521).
